The second installment of the three-part crossover intensifies the search-and-rescue efforts while uncovering crucial leads in the criminal investigation. With time running out for those trapped underground and a fugitive still at large, first responders, law enforcement, and medical teams are pushed to their limits.

At Med, tensions flare between CPD and the hospital staff when Voight and Burgess demand access to an unidentified Jane Doe burn victim, believing she holds key information about the explosion and the missing pension fund hard drive. Dr. Lennox refuses, citing her fragile condition after extensive surgery and the near-certain risk of fatal complications if she is woken too soon. Burgess argues that she may be responsible for multiple deaths and could help save the thirty people still trapped underground, including their own. Frustration builds, and she nearly arrests Lennox for impeding the investigation, but Voight intervenes, recognizing they won’t win this fight.

Later, Lennox takes a risk by sharing confidential medical information that could aid CPD’s case. He reveals that Jane Doe has mesothelioma, a rare cancer often linked to workplace exposure lawsuits. This lead directs detectives to Margaret Simshaw, a former employee of the City Employee Benefits Department—the same office targeted in the heist. Though her last known address is outdated, her connection to the crime strengthens. Meanwhile, a security guard turns in Simshaw’s ID badge, which he found outside the building before the collapse. If she dropped it before boarding the train, she could still be missing—or on the run.

Underground, Lieutenant Kidd and Ruzek continue stabilizing injured passengers while searching for a way to communicate with rescuers. When the train’s signal lights begin flickering in an unusual pattern, they realize it’s Morse code an attempt from Archer to send a message. With no built-in system to reply, a young passenger, Vex, offers an unconventional solution. She and Jacob had built a small robot, Theo, which previously transmitted emergency alerts. She believes she can modify its transmitter to send a signal back to the surface. With no other options, they take the risk, hoping it will restore contact.

Above ground, firefighters struggling to clear debris uncover a hidden duct system that was omitted from official schematics during a building renovation four years earlier. This explains how the offenders bypassed security and accessed the pension fund’s server room undetected. The discovery raises a new concern—one of the burglars is still on the train and may have the stolen hard drive. While apprehending the suspect is critical, the risk of escalating the situation with civilians at risk prevents immediate action. The rescue remains the priority, but oxygen levels inside the train are reaching critical lows.

At Med, Mouch remains by Trudy’s bedside, speaking to her despite her unconscious state. He reflects on their relationship, sharing how he often imagines conversations with her but can never predict what she’ll say next. His voice cracks as he pleads with her not to quit on him now, knowing he wouldn’t know who to talk to without her.

Meanwhile, CPD detectives track down Simshaw’s daughter, Ellie, who recently moved in with her mother. They inform her that her mother isn’t listed among the train passengers or identified victims, meaning there’s still hope. However, Ellie overhears a conversation about Jane Doe and begins to suspect the burn victim might be her mother. When Voight asks for their new address, Ellie refuses to provide it, demanding to see the patient first. Doctors warn that the woman is severely burned and unrecognizable, but Ellie insists she won’t cooperate until she knows for certain.

With oxygen running out on the train, a fugitive still unaccounted for, and Ellie withholding critical information, the situation is at a breaking point. The second installment of the crossover leaves lives hanging in the balance, setting the stage for an urgent and high-stakes conclusion.
