Grey's Anatomy Season 21, Episode 7 Recap: A Tragic Exit, Love, and Big Decisions at Grey Sloan

Season 21 of Grey’s Anatomy continues to deliver emotional twists, and this week’s episode, “The Long Goodbye,” brought both heart-wrenching loss and pivotal life decisions to the forefront. As Yasuda's fate hung in the balance after a devastating car accident with her sister Chloe, the episode also explored some other key character developments, setting the stage for both hopeful and bittersweet goodbyes.


As the episode opened, the aftermath of Yasuda’s car crash played out in heartbreaking real-time. After Yasuda fell asleep at the wheel, both she and her sister Chloe were rushed to Grey Sloan, where doctors and fellow interns scrambled to save them. The sight of Yasuda’s friends and coworkers, including Lucas, in distress as they realized one of their own was fighting for her life was a stark reminder of the fragility of life at Grey Sloan. Yasuda, who had survived the initial trauma and was rushed into surgery, faced severe liver damage that threatened her survival. Teddy, Bailey, Owen, and Ndugu sprang into action to save her, but their efforts were complicated when Lucas had to leave Yasuda’s side to assist with Chloe’s deteriorating condition. It was a frantic, emotional race against time as the interns struggled with their own sense of helplessness.





Meanwhile, a personal struggle for Levi came to a head as he confronted the reality of leaving Grey Sloan. Although he was emotionally tied to James, his desire to move to Texas was growing stronger. Levi wrestled with the decision to leave behind his life in Seattle—his friends, his career, and his burgeoning relationship with James. As he tried to convince himself that long-distance could work, his concerns only deepened when he found himself distracted by the care of baby Ruby, who was briefly in distress. Singing “You Are My Sunshine” to the baby as he performed a procedure, Levi showed how much he had grown as a doctor since his earlier, bumbling days. Jo, ever the voice of reason, reminded Levi that if he was committed, he could make long-distance work. But deep down, Levi knew this decision would change everything.


Later in the episode, Levi made his move and stopped by James’ office to ask the big question: Would he be willing to move to Texas with him? James’ hesitance was palpable, but Levi was resolute. “You’ve lived all over the world, why not San Antonio?” he reasoned. But in the end, when Levi waited for James in the hospital lobby, the meeting didn’t go as planned. James never showed up, leaving Levi feeling discouraged and wondering if he had made a mistake in pushing so hard.


However, Jo, ever the supportive friend, was there to lift his spirits. She cheerfully reminded him of the unexpected and sometimes messy nature of their own relationship, offering her heartfelt support with a special request: Would Levi be willing to be her baby’s godfather? Levi agreed, though the uncertainty of his future loomed large.





As Levi grappled with his personal crossroads, so too did Teddy and Owen. After a rocky few episodes, Teddy made an attempt to reconcile with Owen. She admitted her mistakes and apologized for the kiss with Cass, but Owen remained distant and cold. It was clear that while Teddy had taken the first step in mending their relationship, Owen still had walls up. However, by the end of the episode, Owen admitted that he had overreacted to the situation with Cass and acknowledged his part in the tension between him and Teddy. It was a small but meaningful step forward, though it remained unclear what the future would hold for their relationship.


The emotional rollercoaster hit its peak when Levi, looking out over the hospital as he prepared to leave, saw his entire time at Grey Sloan flash before his eyes. In a bittersweet moment of closure, he rode off on his bike, feeling the weight of all the memories he was about to leave behind. But just as the episode seemed to be closing the door on his story, James appeared, running after him. He had missed their meeting because of administrative duties, but the good news was that he was now ready to take the leap. James had resigned from his position at Grey Sloan and was willing to follow Levi to Texas, marking a happy—but still bittersweet—ending to Levi’s chapter at the hospital.


Back in the ER, Yasuda’s surgery was progressing, but the damage to her liver was extensive. The team worked tirelessly to save her, using an innovative technique to resect parts of her liver and rebuild it using vessels from her spleen. As the surgery unfolded, the interns anxiously waited for updates. Just as the tension began to ease, a code blue call for Chloe was issued. Despite their best efforts to save her, Chloe succumbed to her injuries, leaving the interns devastated. The loss of Chloe, paired with Yasuda’s uncertain recovery, created a profound emotional impact that rippled throughout the hospital.


Days later, Yasuda woke from her coma, and Bailey delivered the devastating news: Chloe had died. Yasuda’s grief was palpable, and as the episode drew to a close, it became clear that the loss of her sister would be a defining moment for her character. As heartbreaking as it was, Yasuda’s story was coming full circle, and it seemed likely that this tragic event would be the catalyst for her eventual departure from Grey Sloan.





With the emotional arcs of Levi, Yasuda, and Owen and Teddy all intertwining, this episode of Grey’s Anatomy offered a mix of hope, heartache, and major decisions about the future. As we prepare for the next chapter, it’s clear that changes are on the horizon for several characters—and the fallout from these events will undoubtedly shape their journeys in the episodes to come. The mix of personal and professional struggles continues to make Grey’s Anatomy as compelling as ever, even as we say goodbye to familiar faces and move toward new beginnings.
